Transitional Care

The Weinberg Manson Transitional Care Program is designed for patients who need short-term, transitional care after hospitalization for an illness or surgery, in order to rebuild their strength and abilities before returning home. The program recently expanded to 68 beds, encompassing the entire second floor of the Jewish Home, along with two restaurant-style dining rooms and a state-of-the-art rehabilitation gym.

We provide a range of services to serve the special and sometimes complex needs of adults, including:
  • Nursing and medical services
  • Physical therapy
  • Occupational therapy
  • Speech and language therapy
  • Orthopedic rehabilitation
  • Bariatric services
  • Pain management
  • Wound care
  • Low-vision and functional-vision services
  • Dietary and recreation services
  • Incontinence therapy
  • Massage therapy
  • Tai chi and stress management classes
  • Social work services

Amenities include:
  • Hotel-quality linens
  • Daily newspaper
  • Spa-like bathrobes
  • Flat screen TVs with free satellite service
  • Wireless Internet access
