The Weinberg Manson Transitional Care Program is proud to achieve excellent clinical outcomes for our patients. We track this through a process measurement tool called Functional Independence Measures (FIMS), which measures the amount of improvement from admission to discharge. Click to see our most recent FIMS outcomes

In addition to measuring our patients' improvements, we also measure their satisfaction with the program through a patient survey. Check out our latest Satisfaction Survey results.

The Weinberg Manson Transitional Care program is accredited by CARF International.
